How to Apply for Arizona Health Care Cost Containment System (AHCCCS)

You can apply online for AHCCCS Medical Assistance, Nutrition Assistance, and Cash Assistance using Health-e-Arizona Plus (HEAplus). You can apply for yourself, your family, or someone close to you.

How to Request Transportation

Medicaid members in Arizona can get non-emergency medical transportation (NEMT) to and from covered health services through their health plan. To arrange transportation, you can:

  • Call your health plan’s Member Services number, which is on your insurance card 
  • Provide the following details:
    • Your AHCCCS ID number 
    • Your appointment date and time 
    • The provider’s name and address 
    • Your pick-up address 
    • The type of vehicle you need 
    • Whether you have any special needs, such as wheelchair accessibility 
  • Schedule your ride at least three days before your appointment
