How to Request Transportation
Medicaid members in Arizona can get non-emergency medical transportation (NEMT) to and from covered health services through their health plan. To arrange transportation, you can:
- Call your health plan’s Member Services number, which is on your insurance card
- Provide the following details:
- Your AHCCCS ID number
- Your appointment date and time
- The provider’s name and address
- Your pick-up address
- The type of vehicle you need
- Whether you have any special needs, such as wheelchair accessibility
- Schedule your ride at least three days before your appointment
